with us. Our credit and cash accounts are set up for contractors only.
To set up either of these accounts with LYF-TYM, you must first
complete the form(s) below. Before submitting your application, please
make sure we are able to service your area by visiting the "locations"
tab on the left menu. Cash Accounts
One of the benefits of setting up a cash account at LYF-TYM is
that we can put you on our mailing list so you receive important product,
promotional and pricing information from our company. It also allows us
to track your sales history and get you the best available pricing.
Simply complete and submit the
Account Information
Form. Once your cash account has been established, you will be contacted
by a LYF-TYM representative.
Credit Accounts
We want to provide you the best and most efficient service available.
Once approved, setting up a credit account at LYF-TYM allows you
more flexibility in purchasing material from our company.
Step 1
- You must first complete and submit
the
Account Information
Form. This will quickly allow us to get you set up in our computer.
Step 2
- Print the
LYF-TYM credit application.
- Complete the application, MAKING
SURE TO SIGN IT in the space provided.
- Fax the application to: 336-996-2770
- Then you MUST mail the original
to:

Kernersville NC 27285
Please note: We cannot establish a credit account for you if we
do not receive the original credit application.
Should we require additional information,
a LYF-TYM representative will contact you. Once your application
has been evaluated, we will notify you as to your account status.
